What is Remote Patient Monitoring RPM

Remote Patient Monitoring (a.k.a. as In Home Telemonitoring in Texas) involves leaving automated, simple, user friendly, vital signs monitors (blood pressure, glucometer, pulse oximeter, and weightscale, etc.) in the patients home to monitor the patient’s health.  This enables the patient and the clinicians to work together closely, to coordinate care, and to prevent complications and exacerbations.  This also helps the patient to continue living independently in the comfort of their own home, thus avoiding unnecessary hospitalizations, and nursing homes, and other forms of institutionalization.

Who qualify for Remote Patient Monitoring?
