Our Remote Patient Monitoring program can be used to deliver healthcare services to patients regardless of their physical location or condition. (PAS) Provider Services is prescribed by the patient’s doctor to support the patient’s health, safety, independence, and improve patient outcome. PAS is an integral part of controlling costs, maintaining the independence of the patient at home, and improving patient outcomes. Americare Nursing Services can achieve this through home health aid or personal assistance to patients whose health problems cause them to be functionally limited and in danger of complications. PAS assists patients with limitations by helping them to age independently at home, thus improving their quality of life.

Provider Services | Respite Care

Stressed families that are caring for aging relatives may find they need an occasional break in order to care for themselves.  Respite care fills this need by providing care over extended periods of time so that the family can receive a much-needed opportunity to recover.  Respite care is available through Personal Assistance Services.

Who may benefit and/ or qualify for (PAS) Provider Services?

Over 70% percent of all Medicare beneficiaries in the Rio Grande Valley (RGV) are dual-eligible (Medicare and Medicaid).  Any patient who has at least partial Medicaid and medical necessity should qualify for some assistance.  This means there are many patients who can benefit from PHC and related services.  PHC and related programs may be able to help prevent serious medical complications and reduce medical costs.

What is meant by Activities of Daily Living?

(PAS) Personal Assistance Services attendants assist with the following:

  • Bathing
  • Dressing
  • Feeding/Eating
  • Grooming, Shaving or Oral Care
  • Routine Hair/Skin Care
  • Toileting
  • Transfer
  • Cleaning
  • Laundry
  • Walking
  • Meal Preparation
  • Escort
  • Shopping
  • Assist with self Medication

Other programs under Home Health Aid/(PAS) Personal Assistance Services

Other programs are: CAS, Family Care, and PCS.  These programs provide similar services, but that may have different criteria for qualification.  Some of these qualifications are listed below.

To qualify for CAS (Community Attendant Services) an Individual must:

  • Have multiple physical limitation
  • Have partial Medicaid (QMB) and Medicare
  • Be approved by the State of Texas
  • Be approved by a physician

To qualify for PCS (Personal Care Services) an individual must:

  • Be a child with disabilities from 2 to 21 years of age
  • Have full Medicaid (MQMB) or Traditional Medicaid
  • Be approved by the State of Texas
  • Be approved by a physician

FC (Family Care) is a Federal Funded Program, these programs, PHC, CAS, PCS, and FC can be paid for through insurance or private pay.

What is Hospice Care?

Hospice care is intended for terminally ill individuals.  A team of highly qualified health care professionals who try to make the terminally ill person more comfortable by administering pain medication and being attentive to their physical, social, spiritual, and psychological needs that may be needed at that time.  Our Hospice services focus on providing the terminally ill person with the highest quality end-of-life experience possible.

Hospices can be the ideal terminal care solution for individuals who are expected to live for less than six months.  However, hospice care can be provided for as long as the terminally ill person’s physician can certify that the medical condition is imminently terminal. Hospice care is administered for people who suffer from all kinds of illnesses. These sometimes include, but are not limited to, cancer, heart disease, AIDS, and dementia. 

Many hospices provide care to the terminally ill at home.  In these cases, a family member works closely with the hospice team to provide primary care.  Hospice care is also available at dedicated facilities, nursing homes, and hospitals. 

Despite receiving hospice care at home, there are times when the terminally ill patient might need to be admitted to the hospital.  In these cases, the patient may exhibit a symptom that the hospice team cannot handle in a home setting.

Who qualifies for hospice care?

Hospice patients have limited life expectancy because of serious medical conditions.

  • Anyone whose treatment options have otherwise been exhausted may find that hospice care is a practical and comforting solution for achieving the optimum care and comfort at the most difficult time in that person’s life.
  • A terminally ill person such as a patient with cancer, heart disease, AIDS, and dementia.
