  • The Patient has a new or existing qualifying diagnosis.
  • The physician is able to establish the condition through ongoing documentation and evaluation
  • All required Insurance documents are completed by referring Physician
  • The Patient’s condition requires special attachments that cannot be fixed and used on an ordinary bed.
  • The patient is being treated for Stage II Pressure Ulcer and has been on a Pressure Ulcer Treatment Program for 1 month.
  • The Patient has a new or existing qualifying diagnosis relating to Lymphedema or Venous Insufficiency.
